Youth in transition program of S.D.G.
Support for youth, ages 16 to 24, leaving the care of Children's Aid Society and transitioning from the child welfare system into adulthood. The goal is to connect youth with the supports in their communities and assist them in their navigation of transition from care to adulthood. Promote communication and link youth with community agencies/organizations to improve access and mitigate barriers to services. Support can be provided in the areas of education, employment, financial, health, housing, legal, life skills, mental health and addictions.
