Fegional, patient-centred, primary health care team set up to provide unparalleled collaborative health care to improve the overall health of the Georgian Bay population. The GBFHT is made up of doctors, nurse practitioners, registered nurses specializing in wound care, diabetic education, and high blood pressure control education, a dietitian, mental health counsellors and administrative staff whose most important goal is to provide patient-centered care. Patients of family doctors in the area have access to GBFHT's services by getting a referral from their GP. Individual and marital counselling available, currently have seven counsellors on staff, which include two child and youth mental health counsellors for short-term mental healthcare. Patients 16 years and under can be helped with anxiety, depression, anger, acting out, conflicts with parents/siblings, addictions, as well as problems with school and socialization. Offers one-on-one or may be counselled with family members present. Must be referred by local family doctors. Services are covered by OHIP.
